New Patient Registration

The Practice has an open list and accepts Patients whose address has one of the following postcodes: S66 7 and S66 8 (i.e. resident in Maltby, Hellaby, Micklebring or Stainton).

The practice has an ‘Outer Practice Boundary Area’, where at the patient’s request we will continue to provide medical services for existing patients (and other household members) who move to addresses in the following areas; addresses with postcodes S65 4.., S66 1.., S66 2..or S66 9.. live at Brookhouse, Laughton or Firbeck.

To register we will ask you to complete a registration form and to make an appointment with our HCA/ Practice Nurse for a new patient medical, (where possible we arrange it for the same day).

Under 5’s will not require a new patient medical, but the Health Visitor will be informed and a visit arranged within the next 2 working days.
